The Ohio Assisted Living Admission Agreement (2002) was drafted by Aric Martin. It provides a sample admission agreement that incorporates the requirements of the newly revised O.A.C. § 3701-17-57. The agreement also provides many provisions that are not required by law, but that are designed to protect an assisted living facilities interests. This document provides guidance on many of the duties imposed on a licensed residential care facility by Ohio's licensure law. Also, it provides sample policies for advance directives, grievance, transfer and discharge, government agencies, and receipt of documents.
